1.11.3 contains PR #1804 content which is for 1.12.0

#1804 introduces error templating

which bloated my LLM retry call with long reasoning content, but that's another story.

The problem is this change is at release note of v1.12.0 but this templating is also on v1.11.3, I don't know why but there are two commits with same change, for each, v1.11.3 and v1.12.0.

• v1.12.0: f753dcd

IMO, 1.11.3 should be yanked (since it is breaking change), or at least, 1.11.3 releas note should contain same PR changelog.

Resolved - This was a historical versioning issue from v1.11.3/v1.12.0 that cannot be changed retroactively. The project has moved forward to v1.13.0.
