Mailchimp Block: Support adding a subscriber name field.

From 2704143-zen, currently, we do not have any option to add a subscriber name field.

It would be great to have this ability for our Mailchimp Block.

The workaround here is to build the form with Mailchimp's HTML form creator instead, but then stylizing that can be more difficult (requires adding CSS). This would be a great change to see in the future!
