fix(build): serve vscode_web env and requirements from non-ignored source files

- rename .env template to env.sample and point index.json at it
- rename requirements.txt to endpoint-requirements.txt to escape global ignore
- fixes 404s that broke the vscode.dev/azure guided deploy
