Using a maxmind city DB should not be hardcoded for maxmind geoip provider

Description

Right now, the MaxMind GeoIP provider has city as the only available MaxMind provider. In our case we would like to use country as the user "location" and have "Unknown Location" mean that the user's country changed.

Whether the provider should use city or country should be an additional configuration option

Great idea! This will require some refactoring, though. We're going to look into that.
