adjust view of locations for easier QAing

We may need to adjust the locations view (the table( because we would like to use it to aid QAing. Some of the columns are not useful right now (ex request a call) and we need to be able to more readily see who created the location to give feedback to them.

Here is none of my patented mockups for reference:

Here is a related request that will also speed up QAing on locations. (much like this request for adding more info at the top of the page when viewing a report: #487 )

Currently there is not much info at the top of a locations report, and to see who created the location and when, I need to scroll to the bottom, and open up a hidden bit to get that information. It would be nice if this could live at the top of a location.

For the proposed location list view, should those additional fields be added to the end of the list? And did you want to keep latest_non_skip_report_date: Latest report that is NOT is_pending_review and does NOT have a skip tag

another bump on this. would love to get the 'header' information at the top of the individual location records. this would make things much easier to QA!
