Country and city list issues

The country and city drop-down menus in the questionnaire have problems.

• They are not alphabetically ordered when you do a search
• United States of America should be at top for ease of access
• There are a lot of "undefined" cities

Fixed and pushed to staging.

• The Country is not alfabetically ordered.
  Pay attention to the end of the list
  

• There are a lot of sceial characters in the state list for different countries

The last country is placed there because this name has a special characters. so it sorted into last place.
And some city or state has a special characters as a default for some countries. I think we can close this issue as I explained via slack channel.

I have found several places with "undefined" and empty items for the "Select" state list
