Shoptools induction signup & tracker

Problem:
Currently, projects don't have a nice way to sign up people to an induction or keep track of who has been inducted.

Solution:
It would be nice to have something that keeps a CSV up to date on our google drive with members who need to be inducted or have been inducted and for which tools. It must have an easy to use form that people can fill in and potentially a search function which allows you search by any of the fields below.

Required fields:
First name
Last name
zid
email
number
Shoptools induction Y/N
Other inductions: Roland, Tormac, etc
