Create Pull Request Template #104
Description
Simple pull request template that:
- Asks people to say what the problem was and how their PR fixes it and how they tested it
- Asks person to add screenshots of UI changes if applicable
- Requests a review from one of Jad, Tvesha, Raj, and Tyler OR ideally if possible: use the CODEOWNERS file and add the exec 2025-26 team to it so it auto requests a review from all of us (don't think this is possible for free)
