docs(plans): add verified evidence-architecture execution plan

Cross-checks a 47-section external audit against real code (file:line for every
claim), corrects the ~5% that was stale/overstated, and sequences the work into
7 waves + a parallel change-control lane, with a first-10-PR critical path.
