Remove fragmentation code and tests that handle reordering #25
Description
As per #20 (comment) and #20 (comment), fragmented messages are delivered over local streams so re-ordering should never occur. Therefore code and tests that handle re-ordering (rather than in-order fragmentation) should be removed or changed.
