Refactor Issue Template #5

Is your feature request related to a problem? Please describe.
In any project, there will be a lot of issues related to refactoring code. We do not have a separate issue template for that. Having a separate template dedicated to just refactoring will allow us to faster create clearer and more descriptive issues related to refactoring.
Describe the solution you'd like
A refactoring issue template with the following sections:
- Description: A clear and concise description of what the refactoring is about.
- Reason: Why is this refactoring necessary? What problem does it solve?
- Changes: What changes should be made in a pull request solving this issue?
- Additional context: Add any other context or screenshots about the refactoring here.
