Late Form on Confirmation page

Description

Once applicants are accepted and have signed the waiver, they should have the option to indicate if they are showing up late. Let's add a time picker to the confirmation page and a reason field after for why the applicant will be late.

Tip

Reference the IH flow!!!

Acceptance Criteria

• Will you be arriving later than [CHECK IN TIME] dropdown
• Time picker for late arrivals
• Reason field for being late
• Form validation is fully functional
