Add Fragment + Fingerprint URL enhancer tab

- New main tab: enhance VLESS/Trojan URLs with cs (cipher suites),
  fm (fragment mask), fp (fingerprint, default unsafe) and editable
  server override auto-filled from the URL
- TLS-aware injection: cs/fm only added when security=tls
- v2ray-style encoding (%20 for spaces, %3A in cs) matching
  PattNG export-to-clipboard output
- Update header, titles and both READMEs; credit PattNG
- Document client requirements: v2rayN 7.24.7+ (Windows),
  PattNG / v2rayNG 2.3.4+ (Android)
