Description: Pagination cursor could become invalid if underlying list changes during iteration.

Acceptance Criteria:

• Cursor remains valid even after bookmark additions/removals
• Tests add/remove bookmarks during pagination
• Results consistent and complete
• Duplicate prevention verified
