Support GA4GH Passport claims

Add support for Support GA4GH Passport claims.

• Support the linking of user identities form different IdP
• Generate Passport Claims per IdP and add them to the ID token return from the userinfo endpoint.
• Support passport claims in the policy evaluation process.
• Document how Passport claims can be used in the policy evaluation process.
