Add cv-form and some form items

It would be good to add a cv-form & cv-text-input and some other form controls somewhere in the app since they are heavily used components.

The only requirement is that it has to make sense in the context of the app. So a random form somewhere would not be good.

Ideas:

• Maybe a form to create notes about the songs? The notes would need to be stored in local storage
