Excluded codes are considered as included codes #678
Description
In FHIRValueSetCompose.java in the FHIRValueSetCompose constructor, excluded codes are added to the include list field instead of the exclude list field.
Impact: The 'act-class' test case in the 'tho' test group (see test cases in https://github.com/HL7/fhir-tx-ecosystem-ig) fails, as it only includes the 7 notSelectable codes in the expansion response, instead of including the 119 codes and excluding the 7 notSelectable codes.
Proposed fix: make use of the unused addExclude() method of the same class.
