Remove any references to any data that is not relevant outside USyd #14
Description
Remove/Update the following:
- Remove KIM de-identifier folder along with history
- Remove API mapping with history and reintroduce it after removing any treatment centre related references
- Reintroduce templates if they contain references to any of the centre/trail names
- Reintroduce the example Jupyter notebooks (in both this and the client libraries repo) after removing any centre related information, including in the output fields
