docs(labels): reconcile label index with current source facit

data/labels/INDEX.md was stale (2026-06-10: listed 1w/1d/4h as absent/0
"pending re-draw"). Reconcile to on-disk base counts (excluding sidecars):
BTC/USD 1M=9, 1w=21, 1d=67, 4h=365 (up/down split included). Drop the
"directories absent" text, point phase-status authority to handoff.md, and
link the 20250506 dedup report + ledger. Docs-only; no source labels,
code, deps, or artifacts changed.
