Feature: Recursive ToC

Allow building a ToC recursively from multiple files


Considerations:
- Should have a special marker, e.g. `` to indicate ToC for pages
- Possibility to only include first-found headlines
- How should indention work? We currently count indention based on heading level in the same document; This possibly adds a new level
