chore(ci): remove no-op workflow-permissions.yml stub (per audit #196) (#514)

The file declared permissions at workflow scope but had no jobs section, so GitHub Actions silently skipped it — it was a no-op. Permissions belong inside jobs that need them, not as a standalone workflow.
