[Docs] Add CODE_OF_CONDUCT.md referenced by CONTRIBUTING.md but missing

Why this matters

CONTRIBUTING.md (Code of Conduct section, lines ~463-469) links to ./CODE_OF_CONDUCT.md as a required read before contributing, but the file does not exist in the repo, so the link 404s and there is no actual code of conduct.

Acceptance criteria

• Add CODE_OF_CONDUCT.md (e.g. Contributor Covenant) at the repo root
• Verify the CONTRIBUTING.md link resolves
• Include a contact/enforcement channel
