feat: render inherited extractors in a single document with root definitions in `MeltanoExtractor` directive

Currently, sphinx-meltano renders a docs page per extractor, regardless of inheritance. This produces significant duplication of root information such as settings definitions and capabilities. Inherited plugins should appear as sections in the same document as their parent, with only added/overridden details rendered.
