[Bug]: Issues with activity: transactions missing and incorrect display

Describe the bug
1st issue:
Some transactions are not showing up in activity.
I can consistently reproduce it with Perps and Predict withdrawals.

2nd issue (reported by @sleepytanya):
Labeling Inconsistency:
The first mUSD conversion is correctly marked as "mUSD Conversion" in the Activity list, but subsequent conversions are labeled as "Smart Contract Interaction".

3rd issue (reported by @sleepytanya):
Duplicate Transactions:
A duplicate transaction appears in the Activity list: for example, when I submit 5 mUSD conversions, 6 transactions show up in the list. This is consistently reproducible (see recording for reference).

Steps to reproduce

1. Have funds on either Perps or Predict
2. Click to withdraw
3. Confirm the withdrawal
4. Go to activity
5. Notice how it doesn't show up while pending nor after confirmed

Reproducing in build 4925 - none of the Predict or Perps txs is shown in the Activity list:

Reproduced for Perps:

• Fund Perps is displayed in both Transactions Activity and Perps Deposit Activity
• Withdraw Perps is only displayed in Perps Deposit Activity, not in Transactions Activity
