chdman with discs that use INDEX 02

Does CHDv5 correctly represent discs with cuesheets containing INDEX 02?

It appears CHDv5 does not work correctly with these discs.

These games need testing against original hardware. As it stands now the core doesn't do anything with any index over 01 even when parsing bin/cue.

If real hardware actually uses the INDEX 02+ tracks for anything in game it will require core modification to the cdrom controller code.
