feat: implement per-code minimum trial thresholds for grand averages
- Replace global min_epochs_threshold with per-code thresholds - Enable flexible inclusion criteria for different conditions - Support collapsed error conditions (codes 110, 210) with pooled trials - Track subject inclusion per condition for modular analyses
