Description
x-b3-sampled can give 1 for emitting samples, 0 for not emitting samples, d for debug samples (/shrug), or be omitted in the case of sample deferring, meaning that it's up to the app to decide whether or not it wants to emit
currently, if the x-b3-sampled header is absent, we're electing to always ship spans. this should be configurable, though
