Database setup for WOLA performance test

DB2 for z/OS setup for ERWW test environment:
Obtain 'small' data model from SVT and put in datasets
Update and debug the DDL and data load JCL
Update and debug the BIND JCL.
DB2 V12 requires changes to the source INSERT statements to include column names (these could default before)
