Address "related" tag IDs are out of order or have significant span in large documents

**Describe the bug**
`related` may have the right mix of annotations, but the instances of the annotations by ID may be mixed wires,... 

**To Reproduce**
- web service usage
- medical/pharma use casefrom openfda.
 
**Expected behavior**
- offsets should only be related to a single POSTAL instance
