Wrong text about where `=` may occur

spec says:

Note that the = sign may appear within the complete string only as part of a list of parameters.

This is not correct. It may appear in the $v=<version>, which is distinct from the parameter list.

Indeed, this seems to be text which didn’t get updated in #4
