Document response function ingression functionality

1. Add minimal worked example to: (1) generate response functions with QSIRecon, (2) average them across participants with MRtrix, and (3) pass the averaged response functions into QSIRecon.
2. Document how to build estimate/apply recon specs.
3. Cover why using a common response function is useful/necessary, with references to Rob Smith's publications and code.
