Abstract the index

Move the logic of sorting index entries from the HTML conversion to the pre-processor. Use generic internal markup to indicate groups and sub-headings.

THEN implement the presentation for HTML (no-change refactor?), make this an option for LaTeX, and implement an index for braille (#1450).
