Fix stale README links

* Point the issues / commits / pulls links at the canonical
  ProfessionalWiki/SubPageList repo. The JeroenDeDauw URLs
  redirect today but won't forever, and the canonical home is
  the org repo.
* Update Jeroen's blog tag link from bn2vs.com (cert
  expired) to entropywins.wtf, where the SubPageList posts
  actually live.
