connecting custom MONAI app package to OHIF

I have created a segmentation model as a MONAI application, and I want to connect it to OHIF as an extension. However, I’m not exactly sure what steps I need to take.
I’d really appreciate it if you could guide me.
p.s: my application has 4 operators and certainly an application class .py file and I have created a package using this command
"monai-deploy package my_app.py --tag seg_app:1.0 --model models/model --log-level DEBUG",
what should I do next?
