Add DDL for Field Resource

For each single or multi-valued lookup, there should be an entry for the "LookupName" in the lookup table.

The Field Resource has changed and has had items added to it since RCP-033 was originally created, and there are some deprecated elements in the Field Resource in 1.7 as a result.

The changes from RCP-042 define the new Field Resource, and this project needs to be updated to support a JSON metadata report using the new format. See RESOStandards/transport#76
