Optionally capture a user's institution at signup

The identity service signup page needs to be extended to allow for optional input of an institution name. In the near term this will be useful in the context of collecting and reporting SADCO data usage statistics. In future the same will likely apply to MIMS, SAEON and potentially SAPRI.
