Identify and Fix Doc Issues

We need someone to read thru the existing Torsion docs here:

https://github.com/SickheadGames/Torsion/tree/master/docs

And write up changes to reflect how Torsion works now in 2016 with the latest T3D and T2D fixes.

Further improvements to docs can come later.
