Repetition of Patient name in TAKE

@kani3012 ,Why is the user have to enter again the patient name in the TAKE section?

@aayusharora this is a field we want people to fill to know why he needs the blood. As the project is in beta phase right now, more will be done with that information.

@kani3012 , we already have the patient name from the first step . This info is only needed to ensure for whom we need the blood . The placeholder "Patient name " seems quite confusing for user to enter which name we are talking about ( logically the patient itsellf will not demand the blood) .
