Include historical address

Patient address is subject to change. Anonlink's ngram matching is pretty flexible, so one way forward might be to combine any historical values for each field. The matching shouldn't be positional. I'm not sure how varying field lengths would affect the match, so that might need to be resolved by a set field length.
