Creating Site Visits error: ManagementForm data is Missing

When creating site visits getting 500 error, saying managementform data is missing or has been tampered with. Likely a problem with rendering the management form in one of the templates.

Update: Looks like it's an issue in dynamically adding forms via common_forms.js actually. Looking in to further.
