[Bug] [CustomCommands] editing a tag's name creates a duplicate

What happens when the bug or crash occurs?

Happens when I edit a tag's name. It creates a duplicate of the tag with the name I tried to change it to.

What is the expected behaviour?

Change the tag's name instead of creating a duplicate of it.

How do you recreate this bug or crash?

1. Create a tag, name it like "test", add whatever to the response box.
2. Change the tag's name to like "test 2".
3. Suddenly you have two tags named "test" and "test 2", with the same content in the response box.
