Add a Slack invite request form

Description

Right now we use a google form for requests to join the slack community (https://docs.google.com/forms/d/e/1FAIpQLSfrhpy8IhbCrWrDkSYsu-X48DnfKt77WfmsDeNovxPnyiCj0w/viewform). People fill it out and are manually added. Chapter leaders also collect information and add people in their own ways.

Why this is valuable:

We'd like to make it easier for people to request to join and easier for admins to manage the process.

Acceptance Criteria

• There is a form to request to join our slack community on our website
• There is a slack notification (or some type of notification for admins) when someone fills out the form so they can be added to slack.
