Describe the bug
Goose is asking permission to run a tool, and presents the choices of "Allow Once", "Always Allow" or "Deny" but does not state what tool it is actually trying to run.
To Reproduce
Steps to reproduce the behavior:
- Start a conversion.
- Set the mode to manual.
- Prompt something that will cause a tool call.
Expected behavior
I expect to see a preview of what tool is being used, or command executed.

- Version: 1.28.0
- Extensions enabled: None / Default
- Provider & Model: Claude Code -> Haiku 4.5
Additional context
This has been happening since I first tried goose and this is a showstopper for recommending it as a general purpose AI UI.
My Claude code instance runs through a custom gateway, however, as far as Goose is concerned, its a typical claude code installation.
