Fan staying on

I'm continuing to struggle with environmental controls. The dew heater seems to be working ok, but my fan is staying on when the settings say it should be off. See attached picture.

I confirmed the fan is physically running when the pictured settings were snapped. The charts and data on the latest image are all consistent in that they say the fan is on -- even though it shouldn't be.

Suggestions?

PS: Looking at the charts, I do see the fan has been cycling some during the day. From charts its hard to determine if it switched when it should have done. I'm not using a PWM controller -- just a simple relay.
