chore: release v6.0.0

BREAKING: removed axiom + web-ctl plugins from the marketplace (26 -> 24
installable). Adds the unified repo-intel core lib (folded repo-map + embed),
skill-patterns string-'true' fix, ReDoS + prototype-pollution hardening, and
drops the stale aiRatio test.
