fix: replace corrupted spec sheet with clean 124-entry PIPELINE-SPEC-SHEET.md

- Removed 7 duplicate entries (supervisor-decision-tests, execute-tool-dispatch-fix,
  pipeline-cli-wiring, agent-ui-device-guard, cpp-sse-streaming, agent-ui-eval-benchmark,
  visual-pipeline-canvas near-duplicate)
- Reduced 6 separators per entry to 1 (751 -> 124 separator lines, ~57KB saved)
- Removed 3 dead footer blocks with inconsistent counts
- Renumbered all entries continuously 1-124 across all 3 sections
- Replaced MASTER-SPEC-SHEET.md and WORK-NARRATIVE.md with PIPELINE-SPEC-SHEET.md
  (124 entries, 3 categories: FEATURE 66, ARCHITECTURAL_UPGRADE 26, BRANCH-ONLY 32)
  and PIPELINE-CONTEXT.md (verifiable code changes only, no narrative fluff)
