Facility for per-user customization: user-installed extensions

Currently it is not possible for individual users to install extensions in a consistent and persistent manner. What _is_ possible is that, once a session has been launched, extensions can be installed (from the coder.io marketplace) and enabled. But that customization has to be repeated for every project and for every session.
