Bug: File tree and editor do not auto-refresh when changes are made externally

Description

The application does not reflect real-time file system changes (additions, deletions, or modifications) made by external applications or Windows Explorer. The changes only appear after restarting the application.

Steps to Reproduce

1. Open a project folder inside the OpenCode application.
2. Open the exact same project folder using Windows Explorer or any external text editor.
3. Create a new file, delete an existing file, or modify a file from the external application.
4. Go back to the OpenCode application.

Expected Behavior

The file tree and active editor inside OpenCode should automatically refresh and sync to display the external changes immediately (Live File Watching).

Actual Behavior

The file tree does not update, and new files do not appear until the OpenCode application is completely closed and reopened.
