Should Assertions require a code?

We need to establish if Subject Assertion rows MUST have a code, or if it's acceptable for them to only have a display. The utility of no-code rows is greatly reduced, but perhaps it could be permitted with a "warning" state or be tracked as a "low quality" measure.

We need to break it down into meeting a couple of user needs:

1. Data should be consistent for tools like the browser to make searching manageable
2. It should be clear when there is additional data that was not / could not be mapped into our model.
