[Bug] Do not display empty diffs and improve instructions

From my colleague Dan Cohn:

> one easy area for improvement while using this a lot - i seem to regularly be getting diffs that make no changes. these should be filtered / instructions improved. 

We should be filtering out empty diffs and streamlining.  We should also consider improving instructions so that this does not happen.
